Dr. Nadarajah cared deeply for his patients and served the Uniontown and surrounding areas with compassion and dedication. He will be greatly missed. Please include the patient’s full name, date of birth, contact information and signature. We will notify you when records will be ready for pick-up. Medical records will be available until August 25, 2025. After that date records will be maintained off site and properly destroyed after such time the Pennsylvania law regarding medical records retention requires.
